What is a Primary Care Network (PCN)?

A PCN is a group of local GP practices that work together to provide coordinated healthcare in your community.

  • Each PCN covers 30,000 to 50,000 patients

  • You stay registered with your own GP

  • You still book appointments and access services as usual


Our Member Practices

  • Knowle House & Tamerton Surgery

  • Lisson Grove & Woolwell Surgery

  • Roborough Surgery

  • North Road West Medical Centre

  • Wycliffe Surgery

Together, we care for around 61,000 patients across Plymouth.


Who Leads the PCN?

  • A Clinical Director (a practising GP) provides leadership

  • A PCN Manager helps develop and improve services for you


What We Do

We work to improve your care by:

  • Offering more joined-up services between practices

  • Providing faster access to care, often from new healthcare roles

  • Supporting people with long-term or complex conditions

  • Helping prevent illness through early detection and personalised care

  • No change to your GP registration

  • Access to more services across the PCN

  • Appointments that may be available outside usual hours

  • Support from specialists like:

    • Clinical pharmacists

    • First contact physiotherapists

    • Mental health workers

    • Social prescribing link workers

  • Join your practice’s Patient Participation Group (PPG)

  • Find PPG details on your GP practice’s website or ask at reception

  • Take part in surveys run by our PCN

  • What works well for you today


Why PCNs Matter

Many appointments don’t need to be with a GP.

By introducing a wider team, we can:

  • Free up GP time for people with the greatest needs

  • Offer longer, more focused appointments when needed

  • Do more to prevent serious illnesses

  • Help you live well at home and in your community

PCNs are part of the NHS Long Term Plan and are backed by national funding to protect and grow general practice.
